Access Domain observed elevated error rates in COAM today between 3:05pm-3:20pm UTC.
The root cause appears to be a MySQL database connection issue.
There were no reports of issues during the timeframe of the incident (likely due to results from the COAM API being cached).
Access Domain will be reviewing the root cause in detail.
